Smoking damages nearly every organ in the human body, from your DNA to your skin, your heart to your reproductive system. Yet quitting, at any age, can start reversing that damage within minutes. Here is what the evidence tells us.
When you light a cigarette, thousands of toxic chemicals are released before a single puff is taken. According to the World Health Organization (WHO), smoked tobacco products contain over 7,000 chemicals, including at least 250 that are known to be toxic or to cause cancer.
Smoking acts as a precipitating factor across almost all major organ systems, meaning it worsens existing conditions, triggers new diseases, and shortens lives.
But while the harm builds with every cigarette, so do the benefits of stopping. In fact, the body begins to recover far sooner than you realise.
Your lungs take the first hit
The most well-known consequence of smoking is its effect on the lungs, and for good reason. People who smoke are 20 times more likely than non-smokers to be diagnosed with lung cancer and are at higher risk of long-term, irreversible lung conditions like chronic bronchitis, adult-onset asthma and chronic obstructive pulmonary disease (COPD).
When cigarette smoke travels through the airways, tar and hydrogen cyanide paralyse the cilia, the tiny hair-like strands that trap and sweep harmful particles out of the lungs. When these are damaged, respiratory infections become far more likely. The smoke then reaches the small air sacs (alveoli), damaging them and potentially leading to emphysema, a form of chronic obstructive pulmonary disease (COPD).
A wide range of smoking-linked respiratory conditions have been identified, including asthma, emphysema, interstitial lung diseases, lung fibrosis, and lung cancer. Nearly all smokers develop respiratory bronchiolitis (inflammation in the bronchioles, the tiniest airways) whether they are aware of it or not.
A direct attack on your heart
Nicotine accelerates the build-up of fatty deposits in artery walls (atherogenesis) causing blockages and narrowing in the arteries, reducing the blood and oxygen supply to the heart. According to the American Lung Association, when cigarette consumption in the U.S. decreased, so did the rates of heart disease, a clear indication of the causal relationship between smoking and cardiac events.
The WHO reports that smokers are 2–4 times more likely than non-smokers to suffer coronary heart disease and stroke. Even adults who don’t smoke but are regularly exposed to second-hand smoke carry a 25–30% raised risk of cardiovascular disease. Passive smokers are also at long-term risk of cardiovascular issues.
Links to cancers throughout the body
Smoking is strongly linked to cancers throughout the body, not only in the lungs. While lung cancer is most commonly associated with tobacco use, smoking is a risk factor for cancers throughout the body. According to the WHO, tobacco use accounts for 25% of all cancer deaths globally and is the primary cause of lung cancer, with smokers up to 22 times more likely to develop it during their lifetime.
Cigarette smoking is responsible for close to 90% of lung cancer cases. The five-year survival rate after diagnosis, even though it has improved in recent years, still remains at only 29.7%.
The Cleveland Clinic notes that arsenic, nickel, and radioactive polonium in cigarette smoke directly damage DNA and block the body’s ability to repair it, setting the stage for cancer and fertility issues.
Confirmed smoking-related cancers include:
- Lung
- Bladder
- Mouth, throat, larynx & oesophageal
- Stomach
- Pancreatic
- Colorectal
- Cervical
- Kidney
- Liver
- Leukaemia
For both cancer patients and survivors who continue smoking, the outlook is significantly worse. They are more likely to develop a second primary cancer.

Vapes are not harmless
Vapes are often presented as the safer choice, but safer does not mean safe. These devices heat liquid into an aerosol that can contain nicotine, flavourings and other harmful chemicals. Evidence links vapes to lung injury, nicotine poisoning, burns and exposure to cancer-causing substances. They can also raise heart rate and blood pressure, expose bystanders to harmful aerosol and pose particular risks for children, teenagers and during pregnancy.
It’s never too late to quit
The most important message in the evidence is this: your body starts to heal as soon as you stop smoking. The WHO timeline below shows how these benefits unfold, from the first 20 minutes to the years that follow:
- 20 minutes — Heart rate and blood pressure drop.
- 12 hours — Carbon monoxide levels in the blood return to normal.
- 2–12 weeks — Circulation improves and lung function increases.
- 1–9 months — Coughing and shortness of breath decrease; cilia regenerate.
- 1 year — Risk of coronary heart disease is about half that of a smoker.
- 5–15 years — Risk of stroke is reduced to that of a non-smoker.
- 10 years — Lung cancer death rate is about half that of a continuing smoker. Risk of mouth, throat, oesophageal, bladder, kidney, and pancreatic cancers decrease.
- 15 years — Risk of coronary heart disease is the same as that of a person who has never smoked.

The Cleveland Clinic notes that while some damage, such as COPD and pulmonary fibrosis, is irreversible, many conditions show meaningful improvement after quitting. If a person stops smoking for several years, their overall risk of cancer and other serious conditions can return to near-normal levels.
